Abstract
To explore the preliminary effect of the coordinated prevention and treatment model of chronic diseases in the integrated health community, taking chronic kidney disease as an example.A total of 578 patients diagnosed with chronic kidney disease during the coordinated prevention and treatment model of chronic diseases were collected from permanent residents in a certain area of the integrated health community in Jiading District, Shanghai as the research subjects. The changing trends of renal function indicators (uric acid, blood urea nitrogen, creatinine and glomerular filtration rate) of patients during the policy implementation period were compared to examine the impact of the policy on patients' renal function.During the implementation of the coordinated prevention and treatment model of chronic diseases, the relevant renal function indicators of patients decreased, among which uric acid, creatinine and glomerular filtration rate showed statistically significant differences. The coordinated prevention and treatment of chronic diseases in the integrated health community is an effective, replicable and popularizable new model of medical and preventive integration and health management in the integrated health community, which is more conducive to effectively controlling the risk factors of patients with chronic diseases, reducing or delaying the progression of the disease, and helping patients improve their compliance with doctors' advice and improve their quality of life.
